The Community Health Assessment (CHA) invites people who live, work, and learn in Montgomery County to share their perspectives about, and experiences with, health and wellness. The results of the survey are one part of painting the picture of our community’s health and wellness needs and priorities and will inform our planning for the future. THE DEADLINE HAS BEEN EXTENDED TO MAY 24, 2024.
The survey is available in English, Spanish, Arabic, Bengali, Chinese-Hong Kong, Chinese-Simplified, Gujarati, Hindi, Italian, Korean, and Vietnamese. Click in the upper right-hand corner of the survey for additional languages.
